Assistance forms are included here for your reference and use. If you are unable to print them, please feel free to contact the Human Services Coordinator, or come by the Select Board’s Office during posted office hours to pick up the forms.
All forms must be printed, completed, and signed by all adult applicants in the household. You must contact the Human Services Coordinator to schedule an appointment, and bring the original, signed forms, and the necessary documents for verification, with you to the intake appointment.

If you have applied for Assistance previously, you will need to print out the Intake From (Form G) and bring any other information requested by the Human Services Coordinator.
